Provide some specific details so we can see what it is that truly excites you. Examples: camping, traveling, going to concerts, snowboarding, writing, painting, cooking

Include your hometown and experiences from childhood. Write your story in a way that you would be comfortable sharing with colleges since we may use some of it in our report.

EXTRACURRICULAR ACTIVITIES AND ATHLETICS

Please list your in-school and out-of-school activities and weekly time commitments. List leadership roles or any special honors or distinctions. After each activity, list the grades in which you have participated or will participate ( 9, 10, 11, 12). Please list these activities/athletics in order of importance to you.
