2010-2011 Enrollment Information

Welcome to the enrollment page for NEW and RETURNING STUDENTS.

Here you will find all the necessary forms and information to complete your enrollment process for the 2010-2011 school year at HPA. For detailed information on specific topics of enrollment, please refer to the Enrollment menu to the left.

Whether you are new to HPA or a returning family, we STRONGLY RECOMMEND that you read through each green tab to make the start of the school year as smooth as possible.

A student is officially enrolled at HPA for the 2010-2011 school year when:

  1. HPA has received the completed 2010-2011 Enrollment Agreement signed by both parents/legal guardians. Return completed and signed agreements to:

    - AND -

  2. The tuition deposit has been paid to HPA ($1,000 for day students or $2,500 for boarding students). The deposit may be paid by credit card (Visa or MasterCard) or wire transfer. For wiring instructions, please contact the Accounting Office at 808-881-4043 (e-mail: accounting@hpa.edu). 

After a new student is enrolled, the Admission Office will issue HPA identification numbers to the student and parents and the Business Office will mail a user name and password to the parents, allowing access to the Parent Information Center.

Tuition Balance

The tuition balance must be paid through Tuition Management Systems. Your Parent ID number is required complete the tuition payment. Please review the Tuition Payment Plans page for more information about payment plans, methods, and deadlines.

  • TUITION REFUND PLAN – The school strongly recommends that parents participate in the Tuition Refund Plan. This plan provides protection in the event of an unforeseen termination of enrollment after the academic year begins. Please review the Insurance/Deposits/Fees page and the Tuition Refund Plan brochure for more information.

Enrollment Forms

To complete the enrollment process, all required forms must be received by HPA no later than June 30, 2010.

Use the links to the right to download the enrollment forms. These are PDF files with form fields so you may complete the forms on-screen with Adobe Reader before you print, sign, and return them to HPA. NOTE: You will not be permitted to save the entries made to the forms.

For your convenience, we have compiled form sets based on grade and status. When you complete the form set on screen, common fields (like Student Name, Grade, etc.) only need to be filled in once.

Here is a brief description of each enrollment form.

  • GENERAL AUTHORIZATION FORM – Required every year for each student.
  • STUDENT INFORMATION/HEALTH AUTHORIZATION FORM – Required every year for each student. Please review the Health Services page for details.
  • FINANCIAL FORM – Required for each student. There is a specific form for day students and one for boarding students. Please refer to the Health Services, Student Bank, Optional Programs, and Insurance/Deposits/Fees pages for help when completing this form.
  • BOARDING STUDENT PERMISSION FORM – Required for each boarding student.
  • PHYSICAL EXAMINATION FORM – A physical examination is required of ALL NEW STUDENTS. An annual physical examination is required of returning students entering grades 3, 6, 9, 10, 11, 12, and postgraduate year. Please review the Health Services page for details.
  • DENTAL EXAMINATION FORM – An annual dental examination is required of all boarding students. Please review the Health Services page for details.

Other Forms

  • REQUEST for ADMINISTRATION/STORAGE of MEDICATION in SCHOOL – Students who take medication while at school are required to inform the school’s health services staff. This form is required for each medication taken. Please review the Health Services page for details. [download this form]
  • AFTER SCHOOL PROGRAM ALTERNATE PICK UP DESIGNEES – This form is optional. Use this form to designate other adults, in addition to those designated as emergency contacts on the Student Information/Health Authorization Form, as authorized to pick up your child from HPA’s After School Program. Update Contact Information

For returning families, please be sure to review/update your contact information at the Parent Information Center. You will need your user name and password to gain access to this secure site. If you need your user name and password reissued to you, please contact the Business Office at 808-881-4019 (e-mail: business@hpa.edu). For security purposes, your information will be mailed to you.

  • ‘OHANA ASSOCIATION DIRECTORY – When you update your contact information at the Parent Information Center, you also will be asked if you would like your information to appear in the ‘Ohana Association Directory. Please indicate your preference in the space provided.
