2012-2013 Enrollment and Registration Information

Enrollment

A student is officially enrolled at HPA for the 2012-2013 school year when:

  1. HPA has received the completed 2012-2013 Enrollment Agreement signed by both parents/legal guardians and submitted electronically* or in hard copy format**. 

    - AND -

  2. The tuition deposit has been paid to HPA ($1,000 for day students or $2,500 for boarding students).  

* To complete the Enrollment and Registration process electronically, please complete the Consent to Use Electronic Signature form and return it to:

** Contact the Admission Office at registration@hpa.edu or 808-881-4321 to request that your child's enrollment and registration packets be sent to you by mail.

Registration Process

Within 48 hours of enrollment, your child’s registration forms will be available online for you to complete and submit electronically when you log in to your Registration Account at https://rws.inresonance.com/hpa. If you requested your child’s enrollment packet be sent to you by mail, you automatically will be sent the registration packet.

Whether you are new to HPA or a returning family, we STRONGLY RECOMMEND that you read through the registration information to make the start of the school year as smooth as possible.

Registration Forms

To complete the registration process, all required forms must be received by HPA no later than June 15, 2012.

These forms include:

  • GENERAL AUTHORIZATION FORM – Required every year for each student.
  • HEALTH INFORMATION AND AUTHORIZATION FORM – Required every year for each student.
  • FINANCIAL FORM – Required every year for each student. There is a specific form for day students and one for boarding students.
  • BOARDING STUDENT PERMISSION FORM – Required every year for each boarding student.
  • PHYSICAL EXAMINATION FORM – A physical examination by a U.S. licensed physician is required of ALL NEW STUDENTS. An annual physical examination is required of returning students entering grades 3, 6, 9, 10, 11, 12, and postgraduate year.
  • DENTAL EXAMINATION FORM – An annual dental examination is required of all boarding students.

Other Forms

  • REQUEST FOR ADMINISTRATION/STORAGE OF MEDICATION IN SCHOOL – Students who take medication while at school that is administered by HPA health services staff are required to inform the school’s health services staff. This form is required for each medication taken.
  • REQUEST FOR SELF-ADMINISTRATINO OF MEDICATION IN SCHOOL – Students who self-administer medication while at school are required to inform the school’s health services staff. This form is required for each medication taken.
  • AFTER SCHOOL PROGRAM ALTERNATE PICK UP DESIGNEES – This form is optional. Use this form to designate other adults, in addition to those designated as emergency contacts, as authorized to pick up your child from HPA’s After School Program. You may designate emergency contacts for your child when you log in to your Registration Account or complete the Student Information/Health Authorization form if you requested a registration packet by mail.
  • UPPER SCHOOL BOARDING ROOMMATE SURVEY – If you will be a freshmen boarder for the 2012-2013 school year or are a new Upper School boarding student, please complete this form.

Update Contact Information

For returning families, please be sure to review/update your contact information when you log in to your Registration Account or complete the Parent Information form if you requested a registration packet by mail.

  • ‘OHANA (PARENT) ASSOCIATION DIRECTORY – When you update your contact information you also will be asked if you would like your information to appear in the ‘Ohana Association Directory. Please indicate your preference in the space provided.
