The Common Application
Students may use the Common Application to apply to approximately 300 colleges and universities. You may download printable forms at this site, or apply online. Remember, in addition to the application, you may also need to complete the following forms: School Report Form, Mid-year Report Form, Teacher Evaluation Forms, and School Supplements.
HPA Profile
This file contains a profile of HPA required by college admissions offices.
HPA Transcript Request Form
Students should use this form to request that the HPA CCC send transcripts to a school.
HPA Permission Slip to Attend a College Information Session
Students should use this form to get teachers' permission to miss class to attend a college information session.
HPA Teacher Input Form
Students should give this form to a teacher who knows them well and ask them to return it to the CCC. This information will be used to write the student's school report for their college applications.
HPA Advisor Input Form
Students should give this form to their advisor ask them to return it to the CCC. This information will be used to write the student's school report for their college applications.
Parent Input Form
Parents are asked to complete this form and return it to the CCC to help the counselors begin to build a student profile.
Completion of this form is required for all international students applying college in the United States.
